Continuity of care report carried out in the vascular surgery nursing consultation at a referral hospital. Pilot test.

Abstract

The Joint Commission on Accreditation of Healthcare Organizations defines continuity of care as “the degree to which the care needed by the patient is effectively coordinated among different professionals and organizations and in relation to time”. There are different strategic lines of intervention that can be implemented to ensure continuity of care, and continuity of care reports are undoubtedly one of the most effective strategies to ensure interlevel communication. The main objective of this study was to improve continuity of care by means of a specific report for patients treated in vascular outpatient clinics who require nursing follow-up in their primary care center. As a specific objective, to assess the opinion of primary care nurses who receive patients from Vascular outpatient clinics with a continuity of care report. The pilot telephone surveys suggest that the continuity of care report is a good starting point for communication between the hospital vascular surgery outpatient nursing office and the primary care nurse who will continue the patient's care.
